Create-abilities Create abilities operates under the Privacy Act 1988 to protect and safeguard all client records in preventing unauthorised use or disclosure of information contained in the record.
Abiding by professional codes of conduct, Create-abilities ensures strict confidentiality when collecting relevant information. The collection of information is important to gain a full understanding of your child/adolescent's history relevant to therapy. However, your consent is obtained prior to gathering information from third parties and exchanging information with third parties, including doctors, teachers, speech pathologists, physiotherapists etc, eg, sending and receiving questionnaires from teachers, sending a copy of your child's report following assessment, liaising with other professionals involved with your child.
Gathering and sharing information with relevant parties involved with the care of your child is important to enable the highest quality of services to be provided.
As a client, you have the right to access your personal information, in accordance with the privacy Act, 1988. |
