Terms and Conditions

All services are to be paid by cheque or cash at the time of the appointment.

Cheques should be made payable to Create-abilities.

Create-abilities is a registered provider with Medicare and recognised with Private Health Funds. It is the consumer's responsibility to pay the full amount to Create-abilities at the time of the appointment, and take their receipt to Medicare or their Private Health Fund to receive a rebate in eligible cases.

A cancellation fee will be charged if a client fails to attend a scheduled appointment. A fee of $50 will be incurred for any assessment scheduled and $35 for any treatment session not attended without providing notice before 8am on the day of the appointment.

Except where exclusion of liability would be contrary to law, Create-abilities will not be responsible for damages or loss of whatever nature or in respect of indemnity claims by other parties arising from delay or interruption caused by: acts of God, legislation, war, fire, flood, drought, failure of power supply, trade disputes, breakdowns, accidents, sickness, failure of external services or any other circumstances beyond the control of Create-abilities.
